What is human in the loop?

In short:Human in the loop means a person reviews and approves certain agent actions before they run. It is reserved for actions that are risky, irreversible or unusual. Done well, it is targeted at a few decisions rather than a stream of prompts that people learn to click through.

How does it work?

Policy marks actions that need approval. The request is held while a named approver decides on another channel.

What does it look like in practice?

Silverfort's MCP Gateway can step a tool call up to a human before it executes.
